Summary

It has become impossible to pretend that the establishment of a diagnostic must necessarily precede any decision made by the physician. But nevertheless, the physician must establish the balance of his toughts over the case he has observed. The DCR the SFMG allows him to do so. It includes over 300 situations that the physician will encounter at least once a year due to the natural law of regular distribution of cases. They are grouped not only in alphabetical order but also according to their diagnostical positions: certified diagnosis or disease picture
(nosologically defined diagnosis) for typical situations, syndrome or symptom for untypical situations. All of them four have the same value to the extent that they delimit correctly the medical problem from the point of view of the function of the general practitioner and permit a clear operational strategy. The list of Avoidable Critical Risks corresponding to each Consultation Result will allow him to act under the best conditions, depending on the other factors determinating the decision, within a space of freedom more or less extended or more or less restricted. Computers greatly facilitate the use of the Dictionary: insertion criteria, Avoidable Critical Risks, diagnostical positions, semiological implements and code follow-up are easily located and selected on the screen.
